So, found one already on line for ya, From Dillion Precision.
Where it says Distance From Centerline, that's where you put the distance in inches that the bullet was off your Point of Aim (POA) by.
Put the distance from the target to the muzzle in , in yards, and it will show the inches, or if you know the inches, just put that in, and put in the sight radius (distance between front sight and rear sight) and SHAZAM, it will show you the correction factor, rounded to the nearest thousandth of an inch.
Here I have screen snapped what it shows when I use the numbers from my previous example, sight radius of 5 inches, distance to target of 25 yards ( I put in the 900 inches on this first one) and the amount the POA/POI was off, 3 inches.

Here it is again, I changed the sight radius to 6 inches, and put in the distance of 75 yards, it calculated the inches (2,700) for me.
