I have only seen Small Pistol Primers in the "Green" .45 ACP

They are becoming more popular and can cause havoc on primer stacks.

Sort your Range pick up brass...

I kind of scanned the thread, and didn't see the answer, but it sounds like you may not have the dies set correctly. As Steve mentioned, have you tried factory ammo? You know I own a bunch of the Lee FCD's. After using them for a while, I have come to the conclusion that it is a solution looking for a problem.

If you need more help then maybe we could get together and see if we can get you going.

I never did come to conclusive proof about what happened. I have shot another 200 reloads through the gun and it never showed back up.

Prior to doing so I uninstalled and cleaned each die, then set them back up from scratch following Lee's instructions. I also added a case gauge check (barrel) into the QC process and shortened COL a bit. Who knows?

I still havent sent the gun back in to deal with the safety issue. Its no longer in the carry rotation, though.
